Participant Release Waiver

• All climbers must have a waiver on file for the current calendar year in order to climb.
• Waivers must be completel with BLUE or BLACK ink only.
• Any climber under 18 must have their waiver filled out COMPLETELY by a parent or legal guardian.
• If a parent/guardian completes a waiver for their child but will not be attending with them, they must provide a photocopy of their valid Driver's License with the waiver.
• When filling out the waiver for your child, ALL fields must be completed. If any portion of the waiver is incomplete then the waiver is invalid and the child will not be able to climb.
